The report went as far as to say that the ship’s crew were “broadly unaware of the passage plan”. They were also not able to alter the navigational decisions made by the pilot even though they had genuine concerns over safety.
“Procedures had not been enacted and actions not taken to ensure the full bridge resources available to the pilot and master were utilised,” the report said.
