An accident report has blamed the poor condition of a Shih Wei bulker’s anchor gear for a collision that damaged a tanker in Australia.
The Australian Transport Safety Bureau (ATSB) said that on 8 May, 2014, in bad weather at Fremantle anchorage, the securing pin worked free from the 18,400–dwt Royal Pescadores’ (built 1997) port anchor chain cable-stopper bar.
The bar then opened and the cable ran out to its end, causing the bitter-end securing arrangement to give way.
