The UK’s Marine Accident Investigation Branch (MAIB) said manager Kyokuyo of Japan had “an inadequate safety culture” in a report into the incident on the 404,053-cbf Nagato Reefer (built 2000) on 9 April 2014 in Southampton.
And the site of the accident had been interfered with by the crew before MAIB investigators attended the scene, it added.
A seafarer was hurt when a lifeboat fell from its davit while being secured, following an abandon-ship drill.
