Canada’s Transportation Safety Board (TSB) blamed “ineffective visual navigation” by the bridge team of a CSL Group bulker for a grounding off Quebec.

The agency said the bridge team on the 39,000-dwt Atlantic Erie (built 1985) relied on buoys for navigation and failed to use all available navigational resources to monitor the ship’s position off Iles de la Madeleine.

That, the TSB said, led to a loss of situational awareness.
