A fumbled navigational manoeuvre and a lack of diligence by a pilot and bridge team led to a collision between a NewLead Bulkers’ panamax and a fishing vessel, according to an Australian accident report.

The 69,000-dwt Grand Rodosi (built 1990) hit the unmanned fishing vessel Apollo S at Port Lincoln in South Australia in October 2010.

According to an investigation by the Australian Transport Safety Bureau (ATSB), the cause lay with a faulty engine operation.
